RESUMO

AIM: Selection of the clinical characteristics of pain syndrome in the orofacial region in temporomandibular joint pain dysfunction syndrome (TMJ PDS). MATERIAL AND METHODS: One hundred and two patients with TMJ PDS were examined using the Verbal Descriptive Pain Rating Scale of the orofacial area. The Clinical Index of TMJ dysfunction (CID) by Helkimo M. was used for objectification of the level of musculo-tonic disorders in the area of the chewing muscles. The psychopathological status of patients with TMJ PDS was assessed with HRDS and SCL-90-R. RESULTS: A significant proportion of patients with TMJ PDS, in addition to algic symptoms, have hypertonus of the facial muscles, the nature and severity of which require not only a special diagnostic assessment, but also follow-up treatment. The analysis of data allowed us to identify three clinical groups of patients: with isolated (muscular-tonic), neurological symptoms; with mixed (combination of muscular-tonic and somatoform manifestations) neurological and psychopathological symptoms and with somatoform symptoms. CONCLUSION: Patients with TMJ PDS are clinically heterogeneous and require clinical differentiation, distinguishing the common group of patients with muscular dysfunction accompanied by pain and emotional response to it (anxiety and minor depressive symptoms) and the group of patients with a somatoform disorder (and manifestations as general anxiety).

AIM: To study a role of psychopathological and social factors in the development of suicidal behavior in patients with different forms of epilepsy. MATERIAL AND METHODS: One hundred and nineteen patients with epilepsy, 57 men and 62 women, mean age 40.7±0.54 years, were studied using neurological and psychiatric examinations, along with a large battery of scales for assessment patient's state and suicidal behavior. RESULTS AND CONCLUSION: Clinical and psychological risk factors for suicidal behavior as well as for motivation of suicide prevention are identified. The formation of psychopathology determines the model of personality changes associated with epilepsy and the patients' response to social exclusion and stigma that lead to suicide.

AIM: To study personality-oriented (reconstructive) and cognitive-behavioral psychotherapy in patients with neurotic anxiety disorders with insomnia. MATERIAL AND METHODS: Ninety-eight patients with disorders coded in ICD-10 as F 40.0; F40.1; F41.0; F41.1; F48.0 were studied. Clinical, psychometric, experimental-psychological methods and polysomnography were used. RESULTS AND CONCLUSION: Phenomenological insomnia types characteristic of neurotic anxiety disorders and their relationship with types of intrapersonal conflicts are described. A quantitative analysis of the dynamics of symptomatic complains, anxiety level, and insomnia is presented. The efficacy of the both psychotherapeutic treatments is shown. However the personality oriented (reconstructive) psychotherapy has demonstrated better and more stable results. This can be explained first of all by its pathogenesis-oriented nature and involvement of the cognitive emotional and behavioral spheres as well as the resolution of the intrapersonal conflicts, which are the cornerstone of neurotic disorders.

RESUMO

Effect of laser radiation with wave length 1,4 microm on exposed cornea of isolated bull eye (in vitro) is studied. Experimental laser coagulator with wave length 1,4 microm based on the semiconductor laser with a fiber radiation output was used in the study. Cornea was exposed at the standardized distance to radiation with different energy settings. Coagulates were morphologically studied, photoregistration and morphometric analysis of obtained images were performed. The depth of radiation absorbtion, type and degree of corneal damage were estimated in the focus and marginal area. The analysis of results shows potential of this laser coagulator to be used for selective effects at different corneal layers. This laser may be applied both for coagulation and therapueutic purposes.

RESUMO

A new technology for nebramycin complex antibiotics isolation from non-filtered culture fluid was developed. The industrial use of the technology permitted to exclude the stage of the culture fluid filtration, to increase the yield by more than 33% and to reduce the time of the process by 1.7 times.

RESUMO

A new laboratory method for isolation and purification of tobramycin by using extraction of a tobramycin derivative with benzaldehyde by methylene chloride, subsequent hydrolysis of azomethine and recrystallization of the formed tobramycin sulfate from solution of sulfuric acid in methanol was developed. The method allows to exclude the stage of chromatographic purification of tobramycin, to reduce the time of the process realization from 120-125 h to 15-20 h, to increase the yield of the target product from 37-40% to 60-65% without decreasing the product quality, to exclude a number of large-size and expensive equipment and to ensure high reproducibility of the technology.

RESUMO

New nutrient media for cultivation of the tobramycin-producing organism were developed. As an additional source of nitrogen the media contain fermentative hydrolysate of the mycelial waste of manufacture of aminoglycoside antibiotics (tobramycin and apramycin). The use of the media provided a 20 to 50% decrease of consumption of soybean meal, an essential food raw material, and design of a low-waste technology for biosynthesis of tobramycin.

RESUMO

The results of 2-year multicenter project "Epilepsy, risk factors and insurance" organized by Epilepsy European Commission, on risk factors deteriorating quality of life of epileptic patients in 9 European countries (Italy, Germany, Spain, Netherlands, England, Portugal, Estonia, Russia and Slovenia), have been used in the study. The "patient-control" pairs were matched for various features characterizing social aspects of epileptic patient's life (educational level, professional level and employment, marital status, driving abilities, sports, medical insurance) as well as medical events (concomitant diseases, medical complications, seizures, frequency of accidents, hospitalization, disablement indices, etc). Patients at low and high risk for quality of life impairment were defined. In Russia, the rate of these risk factors in epileptic patients corresponds in general to that in other European countries.

RESUMO

The quality of life (QL) study has been conducted using the WHOQOL-100 questionnaire in 146 patients with epilepsy in the process of their rehabilitation. Before treatment and restorative interventions, the majority of the patients evaluated their QL as moderate and low depending on the type, frequency and severity of seizures as well as on the age of the disease onset. During restorative therapy, the QL scores increased in physical, psychological and social relationship domains and some indices correlated with the results of restorative treatment.

RESUMO

A clinical pharmacological study (270 patients) was made to optimize therapy of epilepsy with carbamazepines (finlepsin and its retard form). It was established that insufficient (standard) doses of finlepsin are often used, serum levels of the drug are not estimated, drug pharmacokinetics is not taken into account, maximum permissible doses of the drug are not reached. Estimation of the efficiency of both preparations in the treatment of various types of the seizures was carried out. The advantages of finlepsin-retard were demonstrated.

Reviewed in this article are the principle of operation and design features of the pulse wave sensors realizing the method of space-oriented differentiation for the noninvasive arterial pressure meters which can measure pressure during the physical tests of a patient. Disclosed are the test results and procedure for checking the pulse wave sensors using the nonstandard means of measurement.

Since the employment of microwave energy for defrosting biological tissues and for microwave-aided diagnosis in cryosurgery is very promising, the problem of ensuring the match between the contact antennas (applicators) and the frozen biological object has become a pressing one. It is for this reason that investigations were aimed at the study of the dielectric properties of the dog's muscular and fatty tissues in a temperature range of +20 to -30 degrees C including the phase transition of the samples to the solid state. The investigations were performed in a waveguide device using a short-circuit method. During the investigation possible error was estimated. The experiment provides for the reduction of the error caused by the waveguide linear expansion.

A comparative assessment of different methods of angiography for diagnostics of echinococcosis of the liver is given. Conditions of prolonged contrast of vessels and parenchyma of the organ were created by splenoportography and arteriography of the liver with maiodyl which allows to determine the presence and degree of morphofunctional changes and spread of the pathological process in echinococcosis of the liver.
